Mediation Calendar

Upon completion and submission of this mediation request form, a Mediation Agreement will be emailed to each attorney and party via DocuSign, and will include a link for each party to pay his or her share of the mediation fee by debit or credit card. Upon receipt of the signed Mediation Agreement and payment of fees by all parties and attorneys, a ZOOM mediation invitation will be emailed to each party and attorney along with a Mediation Information sheet and a copy of the signed Mediation agreement.
